What axles are you going with? PM AxisT6 he knows the place that makes the conversion plate to mount you disc caliper bracket onto the 14 bolt. Or you can get some spacers made to space your 10 bolt caliper bracket out, thats what he tells me anyway and he is somewhat of a axle expert.

Here's your answer if you want better braking power over the drums:
www.baer.com
Go to page 30 in the application guide that is found under the products bar an their home page.
Part number 4302081
Comes with everything you need to convert the drums to discs. Comes with 2-piston calipers and 14" rotors.
* Will require an 18" rim or larger!
* Just to let you know, if you went back with an 04' O.E. rear rotor set-up, you WILL lose braking power over the current rear drum brake system.

If I had the extra money to spend on a brake upgrade, the Baer conversion kit would be the one I choose. My current mods are holding me back from it right now.
Good luck!

Well, it would have been an option is you were running larger wheels.
From what I was told, the 03-04' braking hardware should bolt on to the 10.5" 14 bolt rear diff after swapping over the backing plates.

Here are some disc brake brackets for a 14 bolt. Don't know if they're what you're looking for.
