my brother had one on his 04 silverado with there hand tuner, I could tell when he took it off and removed the tune(when he had to go to the dealer). He ran it for 3 years and then sold the truck, it worked well

I would go with a custom tune any day

I've got an 03 Avalanche with 195,000 miles, and have run descreened stock and a Jet MAF (screenless) for at least half of those miles. Do the tuning myself. I don't know that the screen was hurting anything, but I'd say not having the screen certainly hasn't hurt anything either.
