The 224 r has more aggressive cam lobes.. Hense the R after the cam number. It has XER lobes and the other has XE lobes

i no 1truess runs a 228r and he has a 5.3 with a T56 with 4.56 gears and if i remember he said he wishs he did a 224 cause its not that streetable if it were me id run the TSP 224/581 112 IMO

The R has more aggressive lobes and will make more power but will be harder on the valvetrain
