how many have INCREASED mpg's with a cam swap in a 6.ol???

nothing you do other than reducing rotational mass or some tuning tricks will increase MPG.
All this other stuff will reduce MPG.
I've changed engines, gone from 3.42 to 4.10, changed tire height and weight, increased compression alone, changed transmissions, and added a cam. I'm religious about calculating gas mileage at each fill up so I know the effect all these mods had.

Obviously your biggest issue is the 26's with the 3.42's. You're going to have to gear up, to get back any kind of mileage. Basically like any lifted 4x4 with larger tires.
As far as mileage with cam. I had a similar 04 sierra cc, it was only a 5.3 liter with 3.42 gears. I was running 275-60-17's with denali wheels-->30" diameter. I went to 3.90 gears and lost a couple MPG, highway which is what I mostly drove. Truck was full bolt-on with a 2500 rpm converter. After cam and LT's, cause I did them at the same time, I picked up the mileage I lost with the gears and maybe a little bit over that.
The truck always got great mileage on the open highway.
