Post all your info here and I will try to keep it updated, along with catch can and routing pics of your vaccum hoses.
I wanted to start this thread to find out why I was having a lean code issue. I Have the oil cap vented through a filter and catch can, but that should be closed off and just have the pcv vented if I am reading correct......

here is mine... the catch can is between the manifold fitting and PCV fitting, which is where you want it.. You don't want any of it vented with a breather if your running a MAF... Also I have the fresh air inlet (Passenger side valve cover) after the MAF which is where you want it. I have the hoses tucked behind the fuel rail so they are out of the way.

I would put the vacuum line back on the intake and valve cover and put the oil filler cap back to stock. I would add a catch can though...IMO, all LS based engines need a catch can on the manifold-valve cover vacuum hose because it is poorly designed and allows oil to enter the intake.

so the pcv is on the driver rear valve cover, you run that to the can then from the can to the intake manifold, then on the passenger side you vent from the valve cover to a open air filter right, and block off the port on the tb?
