Where did you get the o-rings for the manifold, GM wants almost 40 bucks for 8 damn orings for the intake.

Hear is what I did:

MAP Sensor: Pull the orange rubber gromet off and it will reveal a barb type fitting, hose clamp vaccum line from there to a spot in the intake.

Brake Booster Hose: Buy a barb fitting to thread into the runner on the intake and connect. The brake booster has a one-way check valve in it already, you can try to blow through it and see.

EVAP Sensor: I deleted mine, however if you want to keep it, just hose clamp vaccum line to it, and run to a spot on the intake.

Vaccum lines from each valve cover: I bought a Moroso Crank Case Breather Tank and welded two pieces of 3/8" aluminum fuel line on the side opposite of the -12 fitting. Run each valve cover hose to this tank, and run a -12 line to the intake side of your Vortech. This helps create vaccum in the crank case which is a good thing, especially under boost. Or if you want you can plug the -12 fitting and run it as is.
