Where did you get your Lift Plate???

The holes are 5 1/8" side to side and 17 1/2" front to back

Thanks. Is that OD to OD dimensions? I am primarily looking for the six center hole dimensions.

Each hole is 4 3/8"center to center and the 5 1/8 is center to center
