You said this was a level 4 FLT but local trans guy freshened it up and now it is the equivelant of FLT level 5, did he put in GM 5 pinion input carrier and
GM 5 pinion reaction carrier?

Yes, he replaced both with GM 5-pinion carriers

Do you have receipts from FLT showing it was a level 4 and receipts from your local guy showing what he did to the transmission?

I have the reciept from my local trans guy laying around here somewhere. As for the FLT verification, FLT stamps the level number on the trans.
