Didn't get a chance to snap pics yesterday and am leaving from work today for a flight, get back Friday night.
But all of the stuff I list as "new" is brand new, in the original packaging, so if you google the part number, that's what it will look like.

I'd only be guessing. If you are using the rear brakes from an SUV or Avalanche with dual piston calipers, I would bet they'll fit. If you can, measure the diameter and thickness of your current rear rotor and I can measure the powerslots.
