They just call it a "Switch Mounting Plate (Pick-up Only)" and it's included in the Heated Seats accessory kit. I don't know if it will fit the location you're talking about or not, just an idea.

GM part number 19165016 Service Component - Front Seat Heater Switch Mounting Plate, Seat Heater Switch Plate

Service Component for Heater part 17803282, which is the kit I installed. It does not include the switches shown in the picture below, those are a separate part number. I still can't find a picture of that exact part number, but I think that's the one. Looks like this:

Sorry, no I don't. When I installed it I was thinking to myself that I could put this where my 4wd switch is located but I did't hold it up to look to see if it would fit. I bet you could measure the 2 holes and see if they are the same. Or try to move your current cubby to the other hole, try swapping them or something.
