Send your current heads to Texas Speed, have them turned into a 2.5 head (1k)
Run the stock intake if you stay below a 222ish cam
4" cai
3000-3400 converter is ok as a DD imho
3:42-4:10s is good gears for a DD imho
Cams can go from a 212/218 to a 222/226 hell even a 224/230 (i think someone is running that cam already) keep in mind the bigger the cam the more converter, gears needed to help push the motor into the power band. The "best combo" is all in taste of the owner. I DD a 3600 big cam an 4:10s i hated it, but loved it on the track.
Decide how you want your truck to DD, mpg wise, simple crusing, and if you want it as a track queen or a street roller. All this depends on what YOU are willing to put up with
