Cam?
I have a 5.3HO in my truck and Comp suggested a cam with 218-224 at .50 and 605-600 with a 112.
Do you think I will have piston to valve problems with this cam?

I have the part number for the springs already, do you know what length pushrods i would need?
Sorry for the stupid questions, Its been awhile since I have done a motor.
Me and my bro share a 92 GMC with a Nitrous big block, makes 470RWHP N/A on pump gas with a small carb.

You really should be giving Comp all of your engine info and mods, and ask them which pushrods to use. Valvetrain design is their business, and they will make the right recommendation the first time. You don't want to take the wrong advice just because somebody doesn't know the correct base circle of your cam.

Your cam should work fine. I have a 4.8 in my truck with a comp cam that has 564/601 lift with 112lsa and it runs as good as it did when it was factory. I would definitley call Comp Cams back and get their recommendation on push rods. If you wanna hear what one of these cams will sound like I have a exhaust clip on my profile page.

I did give Comp all the info on my truck and told them I DD it, Wheel the **** out of it and tow with it and thats the cam they suggested for me, I gave them the mods that I had done aswell.
When I call Comp to order the cam I will ask what pushrods to get, the springs are good to 650 lift that they suggested for me.

I think the cam recommendation is spot on. There's not a whole lot more you can do with the 5.3L for big truck torque other than spin it faster or swap in a bigger motor, so the higher-than-recommended 218/224 will be a good choice. The 5.38's and high lift are the only two things saving that cam from being a complete dog.
What was the name of your cam grinder over the phone?
The reason I ask is because I recognize the cam grind characteristics to one of my good friends at Comp, so I was curious if you spoke to him.
