Default Carbon problem.

Had a vehicle come into my work they other day that had a knock, sounded like piston slap but never would go away after worm up. I used a bore scoop and looked into the combustion chambers and saw that two cylinders where wiped out. the customer said they had just ran a **** ton of techron thru there fuel system the past few fill ups, more that one bottle at a time. so my question is can excess carbon cause a cylinder to wipe out? a few guys are trying to tell me that the carbon let loose and may have got wedged in between the ring lands and wore out the cylinder. the vehicle only has 50k miles. here is some pics

It looks like the pistons got too hot from detonation or a coolant overtemp event. But hey, anything is possible.
