You'll find Mode 6 data on your scan tool. Some only express it in hexadecimal(if you know hex it's not a problem), but some actually do the conversion for you. I thought I read you said your vacuum was only 15psi at idle. That's too low showing mechanical issues which might result in misfires. Is the engine high mileage? A catch can that fills too quickly is a sign of excessive blowby expecially if your NA. I would do a leakdown to see if you're over the limit. If you have mechanical problems it doesn't make sense to check all this other stuff because you're just wasting time until the mechanicals are fixed if your idle vacuum is only 15.

I had those codes along with running lean in both banks, but since I switched to black bear they have gone. I have a spare MAF, intake, gaskets, TPS, injectors, fuel rail, and other parts if you want to borrow them and see if any of those fix your problem. If they do you can just buy them, if not just give em back. not sure if your up for that
